Responsibilities Provide personal care assistance including bathing, grooming, dressing, and mobility support to clients with varying needs. Assist with activities of daily living (ADLs) such as meal preparation, light housekeeping, and medication reminders. Monitor patient health by observing vital signs, reporting changes, and maintaining detailed care plans. Support clients with mobility using equipment like Hoyer lifts and safe patient transfer methods. Ensure client safety by adhering to HIPAA regulations and respecting residents' rights at all times. Facilitate transportation for appointments or community outings while ensuring client safety and comfort. Document care activities accurately and communicate effectively with healthcare teams regarding patient status. Skills Experience working with individuals with disabilities, neurodevelopmental disorders, or in senior care environments such as assisted living or nursing homes. Knowledge of dementia and Alzheimer's care, memory care strategies, and hospice support practices. Proficiency in patient observation, monitoring vital signs, medication administration, and first aid including CPR certification. Familiarity with manual handling techniques and the use of assistive devices like Hoyer lifts or patient transfer methods. Strong communication skills for engaging with clients, families, and healthcare professionals while respecting residents' rights. Ability to perform light housekeeping tasks such as cleaning and meal preparation to promote a safe living environment. Demonstrated compassion, patience, and adaptability to meet the unique needs of each individual served.
