Job Description

Summary As a Direct Support Professional (DSP)/Caregiver, you will provide essential support and care to individuals in our group homes. Your core skills in caregiving will be complemented by premium skills such as first aid/CPR, observation, and assisting with activities of daily living (ADLs). You will be responsible for ensuring the well-being of the individuals through compassionate care, cooking, and maintaining a clean environment. Your relevant experience in long-term care and/or social work will enhance the quality of life for those you serve, making a meaningful impact in their daily lives. We are seeking a PT person for 32 hours per week We have 2pm to 9pm shifts and 12 hour shifts both in Pierz and Little Falls everyother weekend. Responsibilities Provide compassionate care and support to individuals with disabilities in group home settings. Assist clients with activities of daily living (ADLs) such as bathing, dressing, and meal preparation. Monitor and document observations of the individuals, ensuring their safety and well-being. Administer medications as needed and maintain a clean and safe living environment. Collaborate with healthcare professionals and family members to enhance client care. Qualifications Proven experience in caregiving and direct support roles Certification in First Aid and CPR or ability to obtain Familiarity with home & community care practices Ability to assist with activities of daily living (ADLs) Experience in group home setting a plus Strong observation and care skills Background in long-term care or social work is a plus Cooking and cleaning abilities if you are passionate about making a difference in people's lives and want to be part of a dedicated team at Horizon Health, we invite you to apply today!
