Looking For Full and Part Time Day Shift 8a-1p Serious Applications only Overview Join our dedicated healthcare team as a Home Health Aide (HHA), where you will play a vital role in providing compassionate, personalized care to individuals in their homes. This position offers an exciting opportunity to make a meaningful difference in patients' lives by assisting with daily activities, managing medical needs, and supporting overall well-being. As an HHA, you will work closely with clients, families, and healthcare professionals to deliver high-quality home health services that promote independence and comfort. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor and observe patient conditions, recording vital signs and reporting any changes to healthcare providers promptly. Administer medications and perform basic medical procedures such as catheter care, wound care, and hospice support under supervision. Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient care accurately and maintain medical records securely in compliance with HIPAA regulations. Support patients with memory care needs, including Alzheimer's and dementia care, ensuring a safe and supportive environment. Operate medical equipment such as Hoyer lifts and assistive devices to promote safe mobility and prevent injury. Prepare nutritious meals, assist with feeding when necessary, and handle light housekeeping tasks like cleaning and laundry to maintain a clean, safe living space. Provide emotional support and companionship, fostering a positive environment for clients with developmental disabilities or those receiving hospice care. Work within home & community care settings to deliver personalized services tailored to each patient's unique needs. Follow all safety protocols including CPR certification and proper handling of medical supplies to ensure patient safety at all times. Experience Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated ability to perform patient observation accurately and respond effectively to changing conditions. Familiarity with medical terminology, medication administration procedures, and documentation practices using EHR/EMR systems. Experience caring for individuals with Alzheimer's disease, dementia, or other cognitive impairments is advantageous. Knowledge of hospice care principles and working with patients during end-of-life stages is a plus. Long-term care experience involving patient care planning, mobility assistance using Hoyer lifts or similar equipment, and working with individuals with disabilities or developmental challenges is valued. Certification in CPR is required; additional certifications such as HIPAA training or specialized caregiving courses are beneficial.
