Assist with activities of daily living (ADLs), ensuring residents' comfort and safety at all times. Monitor patient health by observing vital signs, tracking medication schedules, and reporting changes to healthcare professionals promptly. Support clients with meal preparation tailored to dietary restrictions and preferences while maintaining proper food safety standards. Facilitate patient transfer methods using equipment such as Hoyer lifts or patient transfer techniques to ensure safe mobility. Maintain detailed care plans aligned with residents' rights, medical instructions, and personalized goals. Conduct light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to promote overall well-being. Provide transportation assistance for medical appointments or community outings as needed. Follow HIPAA guidelines strictly to ensure confidentiality and protect client privacy at all times. Engage in continuous communication with clients and their families to foster trust and transparency about care progress. Qualifications Proven experience working in home health, assisted living facilities, nursing homes, or group homes; experience in geriatrics or hospice care is highly valued. Strong knowledge of patient observation techniques, medication administration, vital signs monitoring, and first aid/CPR certification. Familiarity with specialized care such as Alzheimer's/dementia care, memory care, autism support, or neurodevelopmental disorders is preferred. Ability to perform manual handling tasks including heavy lifting safely while adhering to proper patient transfer methods. Excellent communication skills for effective client interaction and documentation of care activities. Demonstrated understanding of resident's rights and compliance with HIPAA regulations. Valid driver's license and reliable transportation for client outings or appointments are desirable.
