Duties Conduct patient assessments and coordinate care plans tailored to individual needs, including those with mental health conditions, disabilities, or substance use disorders Facilitate intake processes and discharge planning, ensuring seamless transitions between inpatient, outpatient, and community services Collaborate with healthcare providers, social workers, and community organizations to coordinate resources and support services for clients Maintain accurate electronic health records (EHR/EHR) management in compliance with HIPAA regulations and medical terminology standards Provide behavioral health support through counseling, addiction counseling, or mental health interventions as appropriate Assist with diagnostic evaluations and clinical case management to monitor progress and adjust care strategies accordingly Educate clients and their families about health conditions, treatment options, and available community resources Requirements Prior experience working in hospital environments such as Level I Trauma Centers, ICU units, or pediatric settings is highly desirable Demonstrated knowledge of hospital medicine, emergency medicine, acute care, and post-acute care practices Experience working with children, individuals with disabilities or substance use disorders, and diverse cultural backgrounds Proficiency in EMR/EHR systems, medical terminology, and electronic health records management Certification in Basic Life Support (BLS), CPR, and familiarity with HIPAA compliance standards Strong skills in patient service, client communication, task prioritization, and case management Ability to work effectively within multidisciplinary teams in inpatient or outpatient settings and manage complex patient needs Join us to be part of a passionate team committed to delivering exceptional community-based healthcare.
