Duties Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and personal hygiene to promote independence and comfort. Monitor patients' health by observing vital signs, recognizing changes in condition, and reporting findings accurately to healthcare professionals. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Support medication administration following care plans while adhering to HIPAA regulations to ensure confidentiality. Facilitate safe patient transfers using equipment like Hoyer lifts and proper manual handling techniques. Provide companionship and emotional support, engaging clients in meaningful activities to enhance mental well-being. Maintain detailed documentation of care provided, patient progress, and any incidents, ensuring compliance with all safety protocols including first aid and CPR procedures. Experience Prior experience working with individuals with disabilities, neurodevelopmental disorders, or in senior care settings such as assisted living or nursing homes is highly preferred. Demonstrated knowledge of patient observation, monitoring vital signs, medication administration, and dementia or Alzheimer's care. Familiarity with home health practices including hospice care, home & community care experience, and patient transfer methods. Experience working with individuals with autism or developmental disabilities supports a versatile caregiving skill set. Ability to perform heavy lifting safely using proper techniques and equipment like Hoyer lifts when assisting with patient transfers. Proven track record of maintaining resident's rights while delivering compassionate direct support within group homes or long-term care environments. Certification in first aid and CPR is required; additional certifications related to geriatrics, hospice care, or intellectual disabilities are advantageous.
