Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene. Support patients with mobility and transfers using manual handling techniques and equipment such as Hoyer lifts and patient transfer methods. Monitor vital signs, observe patient conditions, and report changes to healthcare professionals to ensure optimal care. Prepare nutritious meals tailored to dietary needs and assist with medication administration following care plans and HIPAA regulations. Provide companionship and emotional support while engaging clients in meaningful activities to promote mental well-being. Maintain a clean, safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing personal spaces. Support clients during medical appointments or community outings through personal transportation assistance and escorting. Skills Proven experience working with individuals with disabilities, developmental or intellectual disabilities support, or senior care environments. Knowledge of dementia care, Alzheimer's care, memory care, hospice care, and neurodevelopmental disorders. Familiarity with home health procedures including patient monitoring, vital signs measurement, medication management, and first aid/CPR certification. Ability to perform heavy lifting safely and use patient transfer equipment like Hoyer lifts effectively. Excellent communication skills for client interaction, family updates, and collaboration with healthcare teams while respecting residents' rights under HIPAA guidelines. Experience working in group homes, assisted living facilities, nursing homes, or long-term care settings is highly desirable. Strong organizational skills to follow detailed care plans and document observations accurately.
