Job Summary We are seeking a compassionate and dedicated Live-In Caregiver/Medication Aide to provide exceptional in-home care for a client requiring assistance with daily activities and health management. This vital role involves delivering personalized support in a home setting, ensuring safety, comfort, and dignity for individuals with diverse needs, including elderly clients, those with developmental disabilities, dementia, or recovering from illness. The ideal candidate will be energetic, proactive, and committed to enhancing the quality of life for those they serve. Responsibilities Provide comprehensive personal care assistance, including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor patient health by observing vital signs, assisting with medication administration, and recognizing signs of medical emergencies. Assist with Activities of Daily Living (ADLs) such as toileting, feeding, and mobility to promote independence and well-being. Maintain detailed medical documentation in Electronic Health Record (EHR) systems while ensuring compliance with HIPAA regulations to protect patient confidentiality. Support clients with cognitive impairments by implementing memory care strategies and providing Alzheimer's and dementia care tailored to individual needs. Prepare nutritious meals aligned with dietary requirements and assist with light housekeeping tasks including cleaning and laundry to maintain a safe environment. Communicate effectively with clients, family members, healthcare providers, and team members to coordinate care plans and update medical records accurately. Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare environments is highly preferred. Demonstrated knowledge of medical terminology, home health care regulations, and patient care standards. Hands-on experience with patient transfer methods such as Hoyer lifts and working with geriatric patients or individuals with disabilities. Familiarity with EMR/EHR systems for documentation purposes along with experience in medication administration and vital signs monitoring. Certification in First Aid and Basic Life Support (BLS), CPR is required; additional certifications in hospice or end-of-life care are advantageous. Proven ability to handle heavy lifting safely while maintaining patient safety during transfers or mobility assistance. Experience working within home health agencies or providing long-term care services that emphasize compassionate caregiving in a home setting.
