The Aging & Disability Resource Center (ADRC), a program of the NH Department of Health and Human Services (DHHS), collaborates with the Partnership for Public Health (PPH). As New Hampshire's official Aging and Disability Resource Center and NHCarePath Full-Service Access Partner, the ADRC serves individuals of all ages, income levels, and abilities. The program facilitates connections to long-term services and supports (LTSS), family caregiver resources, options exploration, and information regarding Medicare and Medicaid. The Caregiver Specialist / VDC Case Manager works as part of the Aging & Disability Resource Center team for Belknap and Carroll Counties. In partnership with the Veterans Administration (VA) Medical Center, this position helps implement Veteran-Directed Care (VDC), giving Veterans and their caregivers access, choice, and control over home and community-based services through consumer-directed care. Additionally, the role provides direct support, counseling, education, and respite care funding through the New Hampshire Family Caregiver Support Program (NHFCSP) to unpaid family caregivers. This position operates within a public health organization that adheres to CDC guidelines and recommendations.
Responsibilities:
1. Case Management & Participant Support Veteran-Directed Care (VDC): o Provide participant-directed and person-centered case management to a caseload of Veterans in partnership with the VA Medical Center Program Director; serve as the primary point of contact for Veterans on the caseload. Conduct assessments, create person-centered plans of care, develop flexible spending budgets, and assist Veterans in implementing home and community-based care services. Family Caregiver Support Program (NHFCSP): Deliver person-centered assistance, information, counseling, and education to family members caring for unpaid individuals (including frail older adults, adults with disabilities, grandparents raising grandchildren, and individuals diagnosed with Alzheimer's or dementia). Conduct assessments, establish plans of care, develop budgets, and facilitate access to eligible respite care funding and community support services.
Fiscal Agency Coordination:
Collaborate with the designated fiscal management agency (currently Gateways Community Services) to ensure prompt, accurate payment for approved client services and goods.
General Information & Referral:
Serve as part of the ADRC team providing intake, information, referrals, option counseling, and follow-up to clients through home and community visits, telephone calls, walk-in appointments, group sessions, or email. 2. Outreach, Education & Marketing Develop, implement, and lead public outreach and educational presentations within the catchment area regarding ADRC, LTSS, Caregiver, and VDC programs. Collaborate with local providers, community organizations, and team members to identify caregivers and Veterans in need of support. Assist in implementing marketing strategies, updating promotional materials (e.g., flyers, brochures), and representing the agency at community events. 3. Administrative, Compliance & Reporting Maintain up-to-date knowledge of agency programs, local/state human service delivery systems, and subject matter domains across ADRC programs. Maintain accurate electronic client records in accordance with documentation policies and Inform USA (formerly AIRS) standards. Assist the Program Manager / Administration in completing required state, federal, and grant reports (including Navigate system data, Home and Community-Based Visits, and ADRC Outreach & Education Reports). Participate in statewide initiatives relative to ADRC and VDC programs in collaboration with the Bureau of Elderly and Adult Services (BEAS) and the VA. Attend mandatory staff meetings, training sessions, and serve on agency/community committees as assigned.

Education:
Associate's degree preferred, or High School Diploma/GED with equivalent human services experience (Bachelor's degree preferred in social work or related field).
Certifications:
Obtain and maintain certification in Inform USA (formerly AIRS) and Person-Centered Options Counseling .
Experience:
Prior experience in case management, social work, or a related human services field. Direct experience working with Veterans, older adults, family caregivers, and/or individuals with disabilities.
Knowledge:
Working knowledge of local, state, and national resources, as well as the broad human service delivery network.
Key Skills:
Demonstrated skills in active listening, customer service, interviewing, advocacy, care planning, and accurate record-keeping. Excellent interpersonal skills with an ability to work flexibly and collaboratively with diverse groups, agency officials, and clients. Proficient computer skills and comfort with client management electronic databases. Ability to organize, schedule, and present at outreach and public education events.
PHYSICAL & SPECIAL REQUIREMENTS
Travel & Driver's License:
Maintain a valid driver's license, a good driving record, and access to a reliable, fully insured personal vehicle for travel throughout the designated catchment territory.
Background Checks:
Successful completion and acceptance of Criminal Records Check and Bureau of Elderly and Adult Services (BEAS) Registry Check.
Work Schedule:
Ability to work a flexible schedule, which may occasionally include evenings or weekends for outreach events or client needs.
Physical Demands:
o Mobility to sit for prolonged periods, walk, stand, and occasionally reach above/below shoulder level. Ability to lift and carry up to 25 pounds to waist-high level when necessary.
