Job Overview We are seeking a dedicated and compassionate Aftercare Caregiver to join our team on a part time, Monday through Friday schedule is 6+ hours daily. ln this vital role, you will provide personalized support and care to individuals transitioning from more intensive care settings back into their homes or community environments. Your energetic approach will help promote independence, safety, and well-being, ensuring each individual receives attentive, respectful, and comprehensive care. Duties Assist clients with activities of daily living (ADLs), including personal hygiene, grooming, dressing, and mobility support using appropriate patient transfer methods such as Hoyer lifts or manual handling techniques. Monitor patients' vital signs and observe for any changes in health status, documenting findings accurately in care plans while adhering to HIPAA regulations for patient privacy. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption to promote proper nutrition and hydration. Support clients with medication administration following prescribed schedules and safety protocols. Conduct patient observation to identify behavioral changes or health concerns, especially in individuals with dementia, Alzheimer's, or neurodevelopmental disorders. Maintain a clean and safe environment by performing light cleaning duties and ensuring all safety standards are met in accordance with home health and assisted living guidelines. Provide transportation assistance for appointments or community activities while ensuring client safety during transfers and outings. Qualifications Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments. Strong knowledge of patient monitoring techniques, care plans, medication administration, and vital signs measurement. Hands-on experience with dementia care, Alzheimer's care, memory care, hospice care, or caring for individuals with developmental or intellectual disabilities. Ability to perform manual handling tasks including heavy lifting and patient transfers using equipment such as Hoyer lifts. Certified in CPR and First Aid; additional certifications related to healthcare or caregiving are preferred. Excellent communication skills for client interaction and team collaboration while maintaining confidentiality per HIPAA standards. Familiarity with home & community care practices and experience working with diverse populations including geriatrics and neurodiverse individuals.
