Job Description
Ready to move from the bedside to coordinating the care? Partners Assisted Living Services (PALS) helps older adults across the Four Corners stay comfortable, safe, and independent in their own homes. We're part of the Basin Health Companies family, serving Farmington, Gallup, and Grants. We're hiring a Care Coordinator — the person who meets new clients, builds their care plan, and checks in to make sure the care we deliver is the care they actually need. If you've spent time as a caregiver or CNA and you're ready for a role with more ownership, this is a strong next step. What you'll do Complete client assessments, build the care plan, and initiate the service agreement Keep open, ongoing communication with clients and their families Conduct periodic supervisory visits and follow through fully on any question or concern raised Reassess clients and update care plans as their needs change Keep the Office Manager informed of changes in client care, status, and condition Install Lifeline units and troubleshoot related issues Work to resolve service concerns, and document the resolution or follow-up every time Relay changes in care promptly to the right team member Participate in the on-call rotation on assigned weekdays, weekends, and holidays Represent PALS positively with clients, families, referral partners, and the wider health care community What we're looking for At least one year of hands-on experience assisting with activities of daily living (ADLs) — caregiver, CNA, home health aide, or similar Previous supervisory or staff training experience High school diploma or GED preferred Valid driver's license and current auto insurance Strong organizational skills and attention to detail Professional, courteous phone manner Ability to stay calm and effective with a wide range of personalities Working knowledge of Microsoft Office 365 Ability to write clear, grammatically correct correspondence and give concise instruction Physical requirements Sit for extended periods and use hands for typing, writing, and handling objects Stoop, kneel, crouch, stand, and walk as needed Lift, carry, push, or pull up to 50 pounds See clearly at close range and maintain focus on detailed tasks Communicate effectively through speaking and hearing Maintain regular attendance and punctuality Why PALS This is a role where you see the outcome of your work.
