JOB DESCRIPTION

On Call Home Maker (Guarantee 40 hours a week)Basin Coordinated Reports to: Staff Supervisor Position summary: The Homemaker is responsible for providing non-medical services, which permits an individual to continue to live in his/her home. The services provided by the Homemaker are directed towards general housekeeping, meal preparation, shopping, laundry, and personal care tasks in which the client is able to assist in his/her own care. The Homemaker neither administers medication nor assists the client in taking medication; however, the Homemaker may remind the client to take his/her own medications. The Homemaker is competent to provide assigned tasks and has completed a competency evaluation program. Basin Coordinated Health Care, Inc. will maintain documentation that requirements of training and competency testing have been met, including number of hours of instructions, date, name or the individual taking the training, the title, purpose and objectives of class and the name of the instructor. The training of Homemaker will be performed by or under the direction of a licensed professional or an appropriately qualified person. Other personnel from health professions may also be utilized as supplemental instructors.

Required Knowledge, Skills and Abilities:

The purpose is to provide standard information and guidelines regarding Agency requirements for the employment of Homemaker. Selection and employment of Homemaker will be made based on: Must be at least 18 years old. Must have a valid driver's license and auto insurance. Must have adequate transportation available. Sympathetic attitude toward caring for clients in their home. Able to visit the office every week to pick up notices and updates. Demonstrated ability to read, write and understand the English language. Ability to carry out directions or instructions. Maturity and ability to deal effectively with the demands of the job. Each Homemaker will be required to pass a Homemaker competency examination with a score of 80% . Basic elements of body function and changes in body function that must be reported to the supervisor. Housekeeping skills. Care of the sick and disabled, including the special needs population. The physical, emotional and development needs of the populations served by the Agency, including the need for respect for the client, his/her property and privacy. Communication skills. What to report to the HMK Coordinator/Supervisor includes changes in the client's condition or family situation. Observation and documentation of client's status and the care or service furnished. Basic infection control procedures. Client/client rights, including respect for cultural diversity. Principles and practices in maintaining a clean, healthy and safe environment as well as a pleasant one that encourages morale building and self-help. Recognizing emergencies/knowledge of emergency procedures. Principles of adequate nutrition, fluid intake and nutritional problems of the assigned client. Confidentiality of all client information. Appropriate and safe techniques in personal hygiene and grooming which include bed bath, sponge, tub or shower bath, shampoo in sink, tub or bed; nail and skin care, including back rub; oral hygiene and toileting and elimination. Safe transfer techniques, ambulation, and positioning. Occupied bed making. Observation, reporting and documentation of client status. Must be able to lift a minimum of 50 pounds. Ability to avoid being under the influence of any controlled substance or alcohol during work hours. Prepares client/client notes daily and submits them to the office weekly to be incorporated into medical records. Each Homemaker shall participate in at least (12) documented hours of in-service training during a twelve-month period. Uphold company policies and promptly report any misconduct Basin Coordinated Health Care, Inc. will maintain documentation that the requirements of training have been met. Job Duties Personal Care-follow care plan in assisting clients with: Bath (tub or shower) Mouth care Dress Ambulation Transfer Turning/Positioning Skin care (applying lotion, peri-care) Must be able to lift a minimum of 50 pounds

Note:

As a rule, a Homemaker may provide only standby assistance during Personal Care activities initiated by the client. Nutrition-follow care plan in assisting client with 1. Menu planning 2. Meal Preparation 3. Feeding clients by mouth (swallowing and gag reflexes must be intact) Treatments-follow care plan in assisting clients with 1. Catheter care (assisting with peri care, empty/change bag.) Homemaking 1. Laundry, make/change bed 2. Light housekeeping (wash dishes, dust, vacuum, mop) 3. Clean kitchen, bathroom 4. Clean appliances 5. Remove trash Home Maintenance 1. Assess potential in home environmental hazard 2. Remove hazards as assessment indicates Other 1. Arrange transportation to doctor, etc. (transport only if no other alternatives are available 2. Run errands 3. Grocery shopping

HOMEMAKER ATTENDANT CANNOT DO

Nursing duties that require the supervision of an RN 1. Give medication to the client (including aspirin, salves, laxatives or other non-prescribed items.) 2. Take temperature, blood pressure, etc. 3. Tube feeding 4. Change or irrigate a catheter 5. Change colostomy bag 6. Apply/change dressings or ace bandages 7. Give a massage Heavy Seasonal cleaning Move heavy furniture or do heavy lifting Wash walls, woodwork or blinds Scrub floors on hands and knees Painting Climb on anything to clean such things as high cabinets, high windows, etc. Clean windows outside of the client's house Perform work requiring skilled repairperson, for example: light a heater, cleaning out drainpipes Clean expensive or sentimental items Clean up urine or feces of animals Cut hair Cut toenails or fingernails Drive the client's car, except with the written permission of the client and with written Basin office consent. Accept gifts or money from the client.
