- Position Overview
- We are seeking a compassionate and independent Licensed Clinical Social Worker (LCSW) to provide
- in-home psychotherapy services
- to primarily geriatric clients. This is a
- W-2 position
- offering flexibility, autonomy, and the opportunity to grow a caseload over time. This role is ideal for clinicians who enjoy community-based work and building meaningful therapeutic relationships with older adults in their home environment.

- Responsibilities
- Provide in-home psychotherapy to elderly and homebound clients
- Complete timely and accurate documentation (Medicare-compliant)
- Conduct assessments, treatment planning, and ongoing therapy
- Coordinate care with families, caregivers, and providers as needed
- Maintain professional and ethical standards in community-based care
- Requirements
- Active
- LCSW license (New York)
- Experience with geriatric, medical, or home-based populations preferred
- Comfortable working independently and traveling between client homes
- Strong documentation and organizational skills
- Reliable transportation required
