We are seeking a detail-oriented and proactive Wellness and Support Representative to join our growing team at our Flatbush Avenue location. This role is essential in ensuring seamless coordination between members, caregivers, payroll, HR, and intake departments.
Position Summary:
The Wellness and Support Representative supports members and caregivers throughout the lifecycle of services. This role ensures compliance, accurate billing, caregiver support, and overall satisfaction while maintaining operational efficiency in a fast-paced home care environment. We are specifically seeking bilingual Spanish speakers and bilingual Haitian Creole speakers to effectively support our diverse member population.
Key Responsibilities:
Complete Start of Care (SOC) documentation for new members in a timely manner Obtain and process Resume, Continued, and Adjusted Authorizations to prevent service interruptions Review and clear Monday.com dashboard tasks daily to maintain workflow efficiency Conduct pre-billing calls to ensure billing accuracy Perform satisfaction calls to gather member feedback and resolve service concerns Update schedules and master weeks to ensure proper coverage Monitor caregiver payments and coordinate with Payroll and HR to resolve discrepancies Initiate requests for pay rate increases, overtime, and adjusted rates as appropriate Follow up on non-compliant timesheets to ensure timely resolution Facilitate caregiver requests related to pay stubs, tax forms, and demographic updates Monitor cases placed on hold, hospital stays, or requiring vacation coverage Coordinate with the Intake Department for re-enrollment cases or plan-to-plan transfers
Qualifications:
Prior experience in home care, healthcare administration, or case coordination preferred Strong organizational and time-management skills Ability to manage multiple priorities in a fast-paced environment Excellent communication and problem-solving skills Experience with scheduling systems and task management platforms (Monday.com preferred) Proficiency in Microsoft Office and general computer skills Bilingual in Spanish or Haitian Creole required
Pay:
$22.00 - $25.00 per hour
