We are excited to offer an opportunity for a compassionate and driven Health Home Care Manager to join our team. This position plays a critical role in managing comprehensive health care plans for individuals in our housing programs, ensuring they receive holistic and continuous care tailored to their specific health needs. Responsibilities Assess new and existing clients to determine their health care needs and eligibility for health home services. Develop, implement, and regularly update individualized care plans in collaboration with other health care professionals. Coordinate and facilitate communication between the client, family members, medical providers, and other relevant entities to ensure effective management of health care plans. Monitor client health status and progress, making adjustments to care plans as needed to address both acute and chronic health issues. Advocate for clients to receive necessary medical services and support from community resources. Maintain comprehensive documentation of all client interactions, services provided, and outcomes. Educate clients and their families on health management, prevention strategies, and available community resources. Required Skills Strong interpersonal and communication skills to effectively interact with diverse client populations. Solid organizational and time-management skills. Proficient in healthcare management and administrative software. Knowledge of patient care coordination and health information systems. Ability to work effectively in both independent and team environments.
