VNS Health is seeking a compassionate Personal Care Assistant to provide high-quality, in-home support to patients throughout the community. In this role, you will assist with bathing, grooming, dressing, toileting, mobility, and daily living activities while promoting safety, dignity, and independence. You will monitor and report changes in patient condition, follow individualized care plans, and support families and caregivers. Responsibilities Provide compassionate in-home personal care, including bathing, grooming, dressing, and toileting Assist with safe transfers, mobility, and exercise to promote comfort and independence Support activities of daily living such as meal preparation, feeding, and light housekeeping Observe, document, and report changes in patient condition to the clinical care team Follow care plans and infection-control protocols to ensure safety and quality care Offer companionship, emotional support, and respectful communication to patients and families Collaborate with nurses, therapists, and caregivers to coordinate holistic care Maintain patient privacy, dignity, and adherence to VNS Health policies and standards Required Skills Personal care and hygiene assistance Safe patient transfer and mobility support Basic vital sign monitoring Infection control and universal precautions Meal preparation and feeding assistance Home safety assessment and fall prevention Documentation and reporting of patient status Use of adaptive equipment (walkers, wheelchairs) Time management and scheduling Communication with patients, families, and care team
