Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary Monitor patient health by checking vital signs, observing changes in condition, and reporting findings accurately to healthcare teams Support medication management by administering prescribed medications following HIPAA guidelines and care plans Prepare nutritious meals tailored to individual dietary needs and assist with feeding when required Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe living environment for clients Provide companionship and emotional support to reduce feelings of isolation and promote mental well-being Follow established care plans and document all services provided in compliance with healthcare regulations Qualifications Valid certification as a Home Health Aide (HHA) or equivalent credential from an accredited program Experience working in home & community care settings, assisted living facilities, nursing homes, or group homes is highly preferred Knowledge of patient observation techniques, dementia care, Alzheimer's care, memory care, and hospice care practices Ability to assist with complex tasks such as medication administration, vital signs monitoring, and working with individuals with developmental disabilities or autism Strong understanding of HIPAA regulations to ensure client confidentiality at all times CPR and First Aid certification required; additional certifications such as Heavy Lifting or Hoyer lift training are advantageous Demonstrated compassion for seniors and individuals with disabilities; excellent communication skills; reliable transportation is a plus Join us today to become part of a dedicated team that values your expertise!
