Job Description

Addus Family Company seeks a compassionate Hospice STNA for night shifts to provide hands-on end-of-life care in patients' homes. You'll assist with personal care, comfort measures, mobility, and basic clinical tasks while closely observing and reporting changes in patients' conditions. Working within an interdisciplinary hospice team, you support patients and families with dignity and respect, uphold safety and compliance standards, and maintain accurate documentation. Responsibilities Provide bedside hospice care during night shifts in patients' homes or facilities Assist with bathing, grooming, toileting, and other personal care needs Monitor and report changes in patients' condition, comfort, and vital signs Support comfort measures, positioning, and mobility using safe transfer techniques Follow hospice care plans under the direction of RNs and clinical leadership Accurately document care provided and observations in required records or EMRMaintain infection control, safety, and compliance with hospice and state regulations Offer emotional support to patients and families, respecting cultural and spiritual needs Communicate effectively with the interdisciplinary hospice team during handoffs Participate in ongoing training and quality initiatives to enhance patient care Required Skills Hospice care procedures Patient hygiene and personal care Vital signs monitoring End-of-life care support Electronic medical record documentation Safe patient transfer and mobility Infection control practices Basic wound and skin care Communication with interdisciplinary team Family and caregiver support
