Responsibilities Assist clients with activities of daily living (ADLs), including bathing, grooming, dressing, and mobility support using patient transfer methods and manual handling techniques such as Hoyer lifts. Monitor and observe patients' health status, recording vital signs, patient observations, and changes in condition to ensure safety and well-being. Prepare nutritious meals tailored to individual dietary needs, ensuring proper food handling and hygiene standards. Administer medications accurately following care plans and HIPAA regulations, maintaining detailed documentation of medication administration. Support clients with personal transportation needs for appointments or community outings, ensuring safety and comfort during travel. Implement care plans designed by healthcare professionals, including dementia care, Alzheimer's care, memory care, hospice care, and support for individuals with neurodevelopmental or developmental disabilities. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning and laundry to promote a healthy living space. Communicate effectively with clients and their families about care routines, progress, and any concerns to foster trust and transparency. Requirements Proven experience working with individuals with disabilities, neurodevelopmental disorders, or in senior care settings such as assisted living or nursing homes. Knowledge of patient monitoring techniques including vital signs assessment and patient observation. Certification in CPR and First Aid is required; additional certifications such as HIPAA training are preferred. Ability to lift heavy objects safely using proper manual handling techniques; experience with patient transfer methods like Hoyer lifts is highly desirable. Strong understanding of resident's rights, confidentiality standards, and home health regulations. Excellent communication skills to effectively support clients' needs and collaborate with healthcare teams. Flexibility to work various shifts including evenings or weekends as needed; reliable transportation is essential for client visits.
