CareChoice Home Care is seeking a dedicated and organized Home Care Outreach & Sales Specialist to join our growing multi-state team. This role is central to ensuring a smooth and positive experience for new and returning clients across all operational states. As the first point of contact for Medicaid waiver home care inquiries, you will guide families through the intake process, coordinate onboarding, and support client retention initiatives.
Key Responsibilities:
Serve as the primary point of contact for new home care inquiries, ensuring a welcoming and professional first impression. Manage the full intake process from initial inquiry through client onboarding, verifying eligibility and coordinating necessary documentation for Medicaid waiver services. Build rapport with clients, families, and referral sources to foster strong and lasting relationships. Collaborate closely with internal departments—including scheduling, case management, and compliance—to ensure seamless client transitions and service delivery. Support retention goals by identifying clients at risk of discontinuing services and implementing proactive case-saving strategies. Maintain accurate and timely records of all interactions, authorizations, and updates within the CRM system. Contribute to department performance benchmarks and growth targets, participating in strategic retention and marketing initiatives. Occasionally represent CareChoice at external events, outreach programs, or community engagement activities.
Qualifications:
Previous experience in home care, healthcare intake, or customer service preferred. Strong communication and relationship-building skills. Excellent time management and organizational abilities, with attention to detail. Ability to work collaboratively across multiple states and manage competing priorities. Proficiency with CRM systems and Microsoft Office Suite. Bilingual (English/Spanish) a plus.
