ABOUT CARE MANAGEMENT BY NATALIE
(CMN) Care Management by Natalie is a client-centered geriatric care management organization serving the SE Michigan region (Ann Arbor, Ypsilanti, Saline, Dexter, Chelsea). We take a holistic approach to supporting older adults and their families through the aging process. CMN offers comprehensive in-home assessments covering health, functional needs, home safety, nutrition, and support systems; develops care plans in partnership with clients and caregivers; connects clients to resources in the community; and provides ongoing well-being monitoring and strategies to maintain independence and quality of life. We're excited to launch a new service line in home support (direct care) to complement our existing care management services — helping clients receive hands-on assistance with daily living while maintaining our strong values of quality, compassion, and client empowerment.
POSITION SUMMARY
As Caregiver Manager, you'll take ownership of how the home support service line runs day to day — leading and developing the Caregiver Coordinator team, driving hiring for the caregiver roster, and keeping operations and documentation running smoothly. This role combines hiring, team coaching, client oversight, and hands-on care into one position. You'll work closely with CMN and the ownership group to keep the service line running well as it grows.
KEY RESPONSIBILITIES
Hiring:
Review resumes, conduct interviews, and lead orientations for new caregivers and coordinators to keep the team staffed as the service line grows.
Scheduling:
Manage day-to-day scheduling for the caregiver team, resolving coverage gaps and shift conflicts to ensure continuity of care.
Coordinator Development:
Hold regular 1:1 calls with Caregiver Coordinators to coach, support, and develop them in their roles.
Client & Care Oversight:
Check in on how clients and client assessments are going, flagging concerns to care managers; monitor caregiver performance and client feedback to catch issues early.
Operations & Documentation Refinement:
Identify and iron out gaps in existing operational processes and documentation standards, building on the systems already in place. Field Support & Direct Care (as needed): Provide high-quality in-home care during staffing gaps, urgent client needs and on call.
Communication & Coordination:
Serve as the primary liaison with CMN and the ownership group on how the service line is running, and between field staff, care managers, and clients/families.
IDEAL CANDIDATE QUALIFICATIONS
Required:
RN or similar Nursing certification 5+ years experience in caregiving, home health, or similar direct care setting, including prior leadership or supervisory experience Experience hiring, interviewing, and onboarding staff Comfortable coaching and developing team members through regular 1:1s A genuine heart for service; compassionate, respectful, and committed to helping older adults remain safe, independent, and valued Strong communication skills, both verbal and written; able to coach and give feedback with empathy Comfort with scheduling tools, documentation systems, and remote/hybrid communication Self-directed and comfortable with ambiguity; able to make decisions and take ownership without heavy oversight
