Job Description

Job Summary We are seeking a proactive and compassionate Home Care Coordinator to join our dedicated healthcare team. In this vital role, you will serve as the primary point of contact for patients, caregivers, and healthcare providers, ensuring seamless coordination of home care services. Your energetic approach will help facilitate effective communication, manage schedules, and uphold the highest standards of patient care and confidentiality. Duties Coordinate and schedule home care visits by collaborating with patients, families, and healthcare professionals to meet individual needs. Maintain accurate and up-to-date electronic health records (EHR) using specialized patient management software and EMR/EHR systems. Communicate effectively via phone and client communication channels to confirm appointments, relay important updates, and address inquiries promptly. Support medical administrative tasks such as processing health insurance documentation, verifying patient information, and ensuring compliance with health regulation policies. Uphold strict confidentiality standards by managing sensitive patient data in accordance with privacy laws and organizational policies. Assist in onboarding new clients by explaining services, gathering medical history, and coordinating initial assessments. Assist with scheduling meetings for manager Provide exceptional customer service by addressing concerns professionally and ensuring a positive experience for patients and their families. Skills Proven experience working in a medical office or healthcare setting with familiarity in medical terminology. Strong proficiency in Microsoft Office applications including Word, Excel, and Outlook for documentation and communication purposes. Knowledge of health regulation policies related to patient privacy, safety standards, and insurance procedures. Experience supporting medical administrative functions such as appointment scheduling, billing support, and insurance verification. Familiarity with patient management software, EMR/EHR systems, and electronic health records management. Marketing and sales Excellent phone communication skills combined with the ability to handle client inquiries with professionalism and empathy. Healthcare and home care marketing experience (MUST HAVE) Ability to maintain patient confidentiality diligently while managing sensitive information. Strong typing skills for efficient data entry and documentation tasks. Knowledge of health insurance processes to assist with claims submission and coverage verification. Pay:
$17.07 - $23.73 per hour Expected hours: 20.0 - 30.0 per week
