Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and manual handling techniques such as Hoyer lifts. Monitor patients' vital signs and observe for any changes in health status, reporting findings accurately while maintaining HIPAA confidentiality standards. Prepare nutritious meals aligned with individual dietary needs and assist with meal planning and preparation to promote healthy eating habits. Administer medications following prescribed care plans and ensure proper documentation of medication administration. Support clients with cognitive impairments such as dementia or Alzheimer's disease through specialized memory care techniques and patient observation to ensure safety. Provide transportation assistance for appointments, errands, or community outings using personal transportation methods while prioritizing client safety. Maintain a clean and safe environment by performing routine cleaning tasks and adhering to infection control protocols, including home & community care standards. Document all care activities accurately in client records, including patient monitoring data, care plan updates, and resident's rights compliance. Engage in ongoing training related to hospice care, neurodevelopmental disorders, developmental disabilities, autism spectrum disorder support, and other specialized caregiving areas to enhance service quality. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable. Demonstrated knowledge of patient personal care assistance, medication administration, CPR/First Aid certification, and vital signs monitoring. Experience supporting individuals with dementia, Alzheimer's disease, or other neurodevelopmental disorders such as autism or intellectual disabilities. Familiarity with HIPAA regulations and maintaining confidentiality in healthcare settings is essential. Proven ability to handle heavy lifting safely using proper techniques like Hoyer lifts or patient transfer methods. Background in geriatrics or long-term care settings that involve assisting with ADLs and providing compassionate caregiving services.
