Responsibilities Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and personal hygiene to ensure comfort and dignity. Monitor patients' health status by observing vital signs, recognizing changes in condition, and reporting findings accurately. Support medication administration following prescribed care plans while adhering to HIPAA regulations to maintain client confidentiality. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking. Provide transportation assistance for medical appointments, errands, or community activities to promote client independence. Utilize proper manual handling techniques and patient transfer methods, including Hoyer lifts when necessary, ensuring safety for both clients and caregivers. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning and organizing living spaces. Document care activities diligently, update care plans regularly, and communicate effectively with clients, family members, and healthcare teams. Skills Proven experience working with individuals with developmental disabilities, neurodevelopmental disorders, or senior populations in home or community settings. Strong patient observation skills coupled with the ability to monitor vital signs and recognize signs of distress or deterioration. Knowledge of dementia care, Alzheimer's care, memory care strategies, and resident's rights principles. Ability to perform CPR and First Aid confidently; certification preferred but not mandatory upon hire. Familiarity with assisted living facilities, nursing homes, hospice care environments, or group homes is highly desirable. Excellent communication skills to effectively support client needs and collaborate with healthcare professionals. Physical capability for heavy lifting and manual handling tasks required for patient transfers or mobility assistance. Experience administering medications safely in accordance with care plans while maintaining compliance with HIPAA standards.
