Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to promote comfort and dignity. Monitor patient health by observing vital signs, tracking changes, and reporting concerns promptly to healthcare professionals. Support medication administration following care plans and HIPAA regulations to ensure safety and compliance. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Operate patient transfer equipment such as Hoyer lifts and utilize proper manual handling techniques for safe patient transfers. Maintain a clean, safe environment by performing light cleaning duties and adhering to infection control protocols. Document care activities accurately in client records, communicate effectively with team members, and support the development of personalized care plans. Requirements Previous experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Knowledge of dementia, Alzheimer's care, memory care, hospice care, and neurodevelopmental disorders enhances your ability to provide specialized support. Strong skills in patient observation, monitoring vital signs, medication administration, and first aid/CPR certification are essential. Experience working with individuals with disabilities or developmental delays, including autism or intellectual disabilities, is advantageous. Ability to lift heavy objects safely using proper techniques such as patient transfer methods or Hoyer lifts; manual handling experience is important. Excellent communication skills for client interaction and collaboration with healthcare teams while maintaining HIPAA confidentiality. Flexibility to work various shifts in home & community care settings; familiarity with social work principles is a plus.
