Care Coordinator / Supervisor Job Summary The Personal Care Aide (PCA) provides non-medical, in-home support services to clients who require assistance with daily living activities. The PCA promotes client independence, safety, comfort, and well-being while maintaining dignity and respect in the home environment. Essential Duties and Responsibilities

Assist clients with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, and personal hygiene.

Provide assistance with mobility, transfers, ambulation, and positioning as needed.

Prepare and serve meals according to client needs and dietary restrictions.

Assist clients with medication reminders in accordance with agency policies.

Perform light housekeeping duties, including laundry, vacuuming, dusting, dishwashing, and maintaining a clean living environment.

Assist with grocery shopping and errands when authorized.

Provide companionship and emotional support to clients.

Monitor and report changes in a client's physical, mental, or emotional condition to the supervisor.

Maintain accurate documentation of services provided and client activities.

Follow all agency policies, procedures, and confidentiality requirements.

Adhere to infection control, safety, and emergency procedures.

Qualifications

High school diploma or GED preferred.

Previous experience in caregiving, home care, healthcare, or a related field preferred but not required.

Ability to communicate effectively with clients, family members, and agency staff.

Ability to work independently and demonstrate sound judgment.

Compassionate, reliable, and patient demeanor.

Ability to perform physical tasks, including lifting, bending, standing, and assisting clients with mobility.

Physical Requirements

Ability to lift and move up to 50 pounds with or without accommodation.

Ability to stand, walk, bend, stoop, and reach for extended periods.

Ability to assist clients with transfers and mobility needs.

Working Conditions

Services are provided in clients' homes.

Work schedules may include evenings, weekends, and holidays as required by client needs.

Exposure to various home environments and household conditions.
