Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility assistance using proper patient transfer methods such as Hoyer lifts and manual handling techniques Monitor patients' vital signs, observe changes in health status, and report findings accurately to healthcare professionals in compliance with HIPAA regulations Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and preparation Support clients with medication administration following care plans and ensuring safety protocols are maintained Provide companionship and emotional support, engaging clients in social activities to promote mental well-being Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing personal spaces Assist with transportation for medical appointments or community outings, ensuring safe patient transfer methods are used at all times Requirements Proven experience working with individuals with disabilities, developmental or intellectual disabilities support, or senior care in settings such as group homes, assisted living facilities, nursing homes, or home health environments Knowledge of dementia care, Alzheimer's care, memory care, hospice care, and neurodevelopmental disorders such as autism Ability to perform patient monitoring including vital signs assessment and recognizing signs of medical emergencies; CPR and first aid certification preferred Familiarity with care plans, medication administration procedures, patient personal care assistance, and HIPAA compliance standards Strong communication skills for effective client interaction and documentation; experience working with diverse populations including geriatrics is a plus Physical ability to lift heavy objects safely (heavy lifting) and assist with manual handling tasks; experience operating patient transfer devices like Hoyer lifts is advantageous Valid driver's license or reliable transportation for client outings; experience providing personal transportation assistance preferred Compassionate attitude combined with a proactive approach to caregiving that promotes client dignity and independence Join us in delivering exceptional home & community care!
