Job Overview We are seeking compassionate and dedicated Hospice Aides to provide essential support to patients and their families during challenging times. As a Hospice Aide, you will play a vital role in delivering high-quality care to individuals in hospice settings, ensuring comfort, dignity, and respect. This position requires a strong commitment to patient care and the ability to work collaboratively with healthcare teams. Duties Provides direct patient care to patient under direction of the RN and according to the Aide Plan of Care. Provide necessary skills to assist the patient with safe transfers and ambulation per the Hospice policy. Provides necessary skill to appropriately report changes and document pertinent information and care rendered to patient to ensure continuity of care. Practices accepted infection control principles. Provide a clean, safe, and comfortable environment. Promotes positive, supportive, respectful communication to patient/family and other employees. Provides an environment which promotes respect for patient, privacy, and property. Provides skills necessary to assist the patient with proper nutrition and adequate fluid intake. Contributes to the management and efficient operation of the Hospice and demonstrate effective time management skills. Provides skills necessary to perform treatments and procedures according to the Hospice policy. Promotes the Hospice philosophy and administrative policies to ensure quality of care. Utilize EMR or EHR systems for documentation of patient care activities in compliance with HIPAA regulations. Skills Proficient in patient observation and understanding of medical terminology. At least one year experience as a Hospice Aide or assisted living, nursing homes, or long-term care settings is preferred. Familiarity with Hoyer lifts for patient transfers and mobility assistance. Knowledge of home health practices and caregiving techniques for individuals with developmental disabilities. Strong communication skills to effectively interact with patients, families, and healthcare professionals. CPR certification is a plus; training may be provided for the right candidate. Ability to work independently as well as part of a team in various home and community care environments. Successful completion of the Aide Competency Evaluation Skills Checklist and written Skills Test.
