Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using proper patient transfer methods and manual handling techniques. Monitor patients' vital signs, observe changes in health status, and report findings accurately to healthcare professionals. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Administer medications following prescribed care plans and ensure compliance with HIPAA regulations to protect patient privacy. Support clients with memory care needs, including Alzheimer's and dementia care, ensuring their safety and comfort at all times. Maintain detailed documentation of patient care activities, updates on progress, and adherence to resident's rights policies. Provide transportation assistance for medical appointments or community outings while ensuring safety during transfers using equipment like a Hoyer lift when appropriate. Skills Proven experience working with individuals with disabilities, developmental or intellectual disabilities support, or senior care environments such as nursing homes or assisted living facilities. Strong patient observation skills coupled with the ability to monitor vital signs and recognize signs of distress or deterioration. Knowledge of medication administration protocols, first aid procedures, CPR certification, and emergency response readiness. Familiarity with home health practices including hospice care, home & community care experience, and assisting with complex care plans. Ability to perform heavy lifting safely and assist with patient transfer methods such as the Hoyer lift or other mobility aids. Excellent communication skills for effective client interaction while respecting residents' rights and maintaining confidentiality under HIPAA standards. Experience working with individuals with neurodevelopmental disorders like autism or behavioral challenges associated with dementia or Alzheimer's disease.
