Sevita is seeking a Lead Caregiver for our Crestwood location to support individuals in home and community-based settings. In this full-time role, you'll provide hands-on personal care, administer medications, and oversee daily living supports while modeling dignity and respect. You'll lead and mentor a small team of caregivers, ensure documentation accuracy, and collaborate with clinical staff and families. Responsibilities Provide direct daily care and supervision to individuals in a residential setting, ensuring safety, dignity, and comfort. Administer or assist with medications per care plans and document all services accurately and promptly. Support individuals with personal care, mobility, meals, hygiene, and daily living activities. Implement person-centered care plans, behavioral strategies, and therapeutic activities. Monitor physical and emotional well-being, reporting changes to clinical or supervisory staff. Coordinate with nurses, therapists, families, and community partners to support individual goals. Model and promote positive social skills, community integration, and independence. Lead and mentor caregiving staff, providing guidance, training, and shift oversight. Maintain a clean, organized, and compliant home environment following health and safety standards. Participate in ongoing training, team meetings, and quality improvement initiatives.
