Job Summary We are seeking a compassionate and dedicated Personal Care Provider to support individuals with diverse needs in their daily lives. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using proper manual handling and patient transfer methods such as Hoyer lifts. Monitor patient health by observing vital signs, administering medication as prescribed, and reporting any changes to healthcare professionals promptly. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking to promote healthy eating habits. Support clients with memory care, dementia care, Alzheimer's care, and neurodevelopmental disorders by implementing personalized care plans and ensuring their safety. Maintain accurate documentation of patient care activities, observations, medication administration, and adherence to HIPAA regulations to protect client confidentiality. Provide transportation assistance for appointments, errands, or community outings using safe client transportation practices. Ensure a clean and safe environment by performing regular cleaning duties and adhering to infection control protocols. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated experience in patient observation, monitoring vital signs, medication administration, and implementing care plans. Hands-on experience with dementia care, Alzheimer's care, memory care, hospice care, or caring for individuals with disabilities or neurodevelopmental disorders is advantageous. Knowledge of resident's rights, HIPAA compliance, manual handling techniques such as Hoyer lifts, and emergency response procedures including CPR and first aid certification. Familiarity with assisting clients with ADLs and providing personal transportation assistance in community settings. Experience working with individuals across various age groups including geriatrics and those with intellectual or developmental disabilities enhances your ability to deliver comprehensive support.
