Heart To Heart AFH is a 4 Bed AFH In Racine WI. We provide total care services to our residents by assisting them with Activities of Daily Living such as self-care, medication administering, clothing/bathing, food/meal prep, leisure time/social companionship, travel to and from appointments as needed, completing accurate documentation for shift rotating communication, personal and social adjustment skills in a home environment.

Responsibilities:

• Provide compassionate and personalized care
• Assist with activities of daily living, including bathing, dressing, and grooming
• Administer medications according to prescribed schedules and document accurately
• Monitor and report changes in clients' health or behavior to the appropriate healthcare professionals
• Assist individuals with developing and maintaining daily living skills
• Provide support and companionship
• Assist with housekeeping tasks, such as cleaning and organizing
• Drive clients to appointments or outings as needed

Experience:

• Previous experience in caregiving or a related field is preferred
• Knowledge of HIPAA regulations and ability to maintain client confidentiality
• Experience working with individuals with autism or dementia is a plus
• Familiarity with medication administration protocols is desirable
• Ability to perform heavy lifting when necessary

Skills:

• Strong interpersonal and communication skills to effectively interact with clients, families, and healthcare professionals
• Compassionate and patient demeanor when providing care for individuals with diverse needs
• Ability to adapt to changing situations and prioritize tasks accordingly
• Attention to detail in documenting client information and following care plans
