Job Summary Community Horizons is assisting members in several different counties throughout the state of Wisconsin. In this vital role, you will provide compassionate, personalized care to individuals in their own homes, promoting independence, safety, and well-being. This position offers an opportunity to build rewarding relationships while empowering clients to live comfortably and confidently in their own spaces. Responsibilities Assist clients with daily activities such as bathing, dressing, grooming, and mobility to support their independence. Prepare nutritious meals and help with light housekeeping tasks to maintain a clean, safe living environment. Provide companionship and emotional support to reduce feelings of loneliness and promote mental well-being. Support clients in engaging with community activities or hobbies that enhance their quality of life. Experience Previous experience in home health care, personal caregiving, or related roles is preferred but not required. Strong interpersonal skills with a genuine desire to help others thrive in their daily lives. Ability to follow detailed care instructions accurately and maintain confidentiality at all times. Reliable, punctual, and Honest Basic knowledge of health and safety standards relevant to home caregiving roles. Valid License and reliable transportation. We look forward to hearing from you.
