QDDP/CASEWORKER

The case worker helps individuals with, but not limited to, crisis situations, manage caseloads, provide referrals, and maintain written documentation.

They also:

Conduct assessments. Assist in developing service plans. Coordinate services. Provide advocacy and support. Work closely with Individuals, their families and QIDPs, to identify needs. Help Individuals access services, including counseling, or financial assistance. Provide individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses as applicable. Advise family caregivers. Provide Individuals with information and counseling. Make referrals for other services. Monitor visits between at-risk or fragile children and family members. Motivate individuals to participate in rehabilitation programs. Report abuse. Case workers should have the following skills: Communication skills Emotional skills Interpersonal skills Organizational and coordination skills Problem-solving skills Time management

Qualifications:

Bachelor's degree in a behavioral science discipline such as Social Work, Sociology or Psychology. Alternatively, a bachelor's degree in science or arts.
