Job Description

Fundamental Objective:
This is an entry level position for individuals seeking to expand their career growth.

Under the direct supervision of the Case Management Supervisor, the Case Manager position will provide direct case management services to individuals and families that are homeless in the Kern County area. The Case Managers' focus is to support these families and individuals, linking them to the resources and aid necessary to obtain permanent housing and self-sufficiency.
Minimal Qualifications:
Possession of an B.A. or B.S. in Social Work/Sociology, Psychology, Human Services, or a related field is preferred, and reduces the required experience to one (1) year, and or two (2) years of experience sufficient to obtain the knowledge and skill to successfully perform the essential duties of the job. 2 years free of domestic violence Valid California Driver's License, car insurance and provide own transportation. Bilingual English/Spanish preferred Must be available to work weekends as needed Must have a flexible schedule.
Essential Job Duties and Responsibilities:
Knowledge of general office practices. Ability to work with clients from diverse socioeconomic backgrounds. Provide on-going comprehensive case management services for shelter residents through positive interactions and rapport building techniques. Conduct needs assessment of clients to develop an individual case plan based on the individual needs of each clients participating in program. Advocate for program participants as they search for supportive and/or independent housing and provide oversight of internal housing programs. Work with clients on a one-on-one basis to provide education, guidance and referrals tailored to each client's unique circumstances and create housing stability plans. Help participants create support systems and participate in the community in which they reside. Assist families with coordinating a move out plan. Assist and coach clients with job search, job placement, interview preparation, resume modification, and promote job retention Work as a team with other ODN staff to provide wellness groups, educational classes, and daily life skills, including facilitating groups. Maintain relationships with services providers, volunteers, staff and community members, which includes attending and representing TODN at community events and collaborative committees as needed. Maintain regular contact with the supervisor and shelter support staff to communicate resident progress and to assist in the resolution of potential problems. Understands the basics of trauma-informed care when working with clients. Attend all staff trainings and meetings for personal and professional growth as assigned. Provide information to clients of their rights and the voluntary nature of our program. General knowledge of (HMIS) Homeless Management Information System to enter/exit client information and data. Daily, weekly, and monthly data reporting and analysis of client services Maintain accurate and up-to-date records of intakes, case notes, and other relevant documentation in compliance with legal and ethical standards. Regular auditing of case files in both digital and physical formats Able to assist with client transports to and from scheduled and non-scheduled appointments in company vehicles. Able to assist with client move-outs from shelter environment to permanent housing. Other duties as assigned.
