Case Manager Assistant - Full Time Peninsula Post Acute is seeking a compassionate, organized, and dependable Case Manager Assistant to join our team. This is a full-time position with a competitive hourly rate and benefits.

Comprehensive benefits package available Position Overview The Case Manager Assistant will provide administrative and coordination support to the Case Management team while helping ensure residents and their families receive timely, compassionate, and effective support throughout their stay. The ideal candidate is highly organized, detail-oriented, professional, and comfortable communicating with residents, families, healthcare professionals, and interdisciplinary team members. Key Responsibilities Assist the Case Manager with daily administrative and coordination tasks Support communication with residents, families, healthcare providers, and other members of the care team Assist with discharge planning and coordination of services Help gather and maintain accurate documentation and records Assist with referrals, appointments, transportation, and other post-discharge needs Coordinate information between departments and outside healthcare providers Follow up on outstanding items to help ensure timely completion Maintain confidentiality and comply with HIPAA and facility policies Provide compassionate and professional customer service to residents and families Perform other duties as assigned to support the Case Management department Qualifications Previous experience in healthcare, skilled nursing, rehabilitation, case management, social services, or a related field preferred Strong organizational and communication skills Excellent attention to detail and follow-through Ability to manage multiple priorities in a fast-paced healthcare environment Proficiency with computers and standard office applications Ability to work effectively with residents, families, and interdisciplinary teams Professional, compassionate, and team-oriented approach Why Join Peninsula Post Acute?
