Full-Time About the Role We are seeking a caring and dedicated Hospice Case Manager LVN to provide compassionate end-of-life care to patients and their families. Under the supervision of the RN Case Manager, the LVN plays a vital role in delivering skilled nursing care, managing symptoms, and ensuring patient comfort in accordance with hospice standards. Responsibilities Provide skilled nursing care to hospice patients in their homes or care facilities. Monitor and record patients' vital signs, symptoms, and responses to treatment. Report any changes in condition to the RN Case Manager and physician promptly. Assist in implementing and updating each patient's plan of care . Administer medications and treatments as ordered by the physician. Educate patients and families on medication use, comfort care, and symptom management. Collaborate with the interdisciplinary team (IDT) including RNs, social workers, chaplains, and aides. Maintain accurate, timely, and complete documentation per agency, state, and Medicare requirements. Support patients and families with empathy, dignity, and respect through all stages of hospice care. Qualifications Active LVN license in the State of California (required). Minimum 1 year of nursing experience , preferably in hospice, home health, or palliative care . Knowledge of pain and symptom management. Excellent communication and organizational skills. Reliable transportation, valid driver's license , and auto insurance . Preferred Hospice or palliative care experience. Familiarity with D ata Soft Logic Bilingual (English/Spanish or English/Tagalog) preferred.Job Type:
