We are looking for a compassionate and resourceful Case Manager to support families through individualized enrollment and stabilization services. This long-term contract position offers the opportunity to work onsite with clients who need guidance accessing employment, education, and financial support resources. The ideal candidate brings a strong service mindset, excellent follow-through, and the ability to build trust while helping clients move toward greater stability and self-sufficiency.
Responsibilities:
- Meet with families to evaluate immediate needs, identify barriers, and develop tailored action plans that support long-term stability.
- Guide clients toward appropriate education, workforce, and community programs that align with their goals and circumstances.
- Provide practical employment support by helping clients strengthen resumes, prepare for interviews, and improve workplace readiness.
- Lead or support financial capability sessions focused on budgeting, money management, and access to public assistance programs.
- Maintain accurate case records, update client progress in required databases, and ensure documentation is completed in a timely manner.
- Coordinate follow-up services with internal teams and external partners to help clients remain engaged in their plans.
- Deliver attentive, client-centered service while responding to questions, resolving concerns, and advocating for family needs.
