March 4, 2026 Position Summary The Resident Service Coordinator (RSC) provides comprehensive case management and supportive services to Project New Hope residents living in permanent supportive housing. The RSC assists residents in maintaining housing stability, achieving self-sufficiency, accessing community resources, and improving their overall quality of life through individualized service planning, advocacy, and ongoing support. Essential Duties and Responsibilities Complete comprehensive intake assessments for all new residents prior to move-in. Develop and implement an Individualized Service Plan (ISP) within 30 days of move-in and update the plan as needed based on resident progress and changing needs. Conduct face-to-face or approved monthly contacts with each assigned resident to assess needs, monitor progress, and provide supportive services. Provide ongoing case management, crisis intervention, advocacy, and referrals to appropriate community resources. Develop, maintain, and strengthen partnerships with community service providers, including agencies offering mental health services, substance use treatment, medical care, employment services, financial assistance, benefits advocacy, and in-home supportive services. Coordinate services with community providers and monitor the effectiveness of referrals and resident participation. Maintain accurate, organized, and confidential resident files in accordance with agency policies and funding requirements. Document all resident contacts, assessments, interventions, and referrals within one week of service delivery. Complete all required monthly case notes in the agency's client database no later than the 5th business day of the following month and maintain hard-copy or electronic files as required. Collect and maintain all documentation required by funding sources, regulatory agencies, and program contracts. Assist residents in applying for and maintaining public benefits, healthcare, and other supportive resources. Provide employment-related support, including resume development, job readiness, employment referrals, and vocational resources. Inform residents of educational, employment, and training opportunities available within the community. Plan, organize, and facilitate resident engagement activities, workshops, and community-building events that promote independent living, wellness, and social connectedness. Design and implement individual and group educational programs that support life skills, housing stability, financial literacy, health, and self-sufficiency. Prepare reports, assessments, and documentation required for program monitoring, audits, and funding compliance. Participate in staff meetings, trainings, case conferences, and interdisciplinary team meetings. Participate in the agency's on-call rotation as assigned. Perform other duties as assigned to support agency operations and resident success. Required License Valid California Driver's License. Proof of current automobile insurance and reliable transportation. Qualifications Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred. Minimum one year of experience providing case management or supportive services to individuals experiencing homelessness, mental illness, substance use disorders, HIV/AIDS, or other vulnerable populations preferred. Knowledge of trauma-informed care, harm reduction, motivational interviewing, and housing-first principles preferred. Strong interpersonal, communication, and organizational skills. Ability to establish and maintain professional relationships with residents, community partners, and multidisciplinary teams. Ability to work independently while managing multiple priorities and deadlines. Knowledge of community resources, public benefits, and social service systems. Proficiency in Microsoft Office Suite and electronic case management systems. Bilingual in English and Spanish is required. Physical Requirements Ability to travel between multiple housing sites. Ability to lift up to 25 pounds on occasion. Ability to sit, stand, walk, bend, and climb stairs as necessary to perform job duties.
