ECM & Community Support Case Manager

The Case Manager supports members from multiple populations of focus with case management deliverables, managing their own health by delivering CalAIM services.

Responsibilities include:

Work under the supervision of the CalAim Program Manager to ensure proper escalation of cases and coordination of care. Serve a caseload of 30 to 60 patients. Provide CalAIM Enhanced Care Management and Community Support Referral Services to all assigned patients. Conduct outreach to each assigned Member and engage each assigned Member to enroll into ECM. Develop patient's individualized Care Plan and complete health needs assessment. Link members to local, county and state services/resources. Build rapport with members by communicating personal experiences with a goal of increasing the likelihood of positive behavior changes. Coach members to create and implement strategies that minimize risks associated with the identified common health conditions and behaviors. Connect members to appropriate programs to address barriers to care and to enhance compliance Follow up with members and serve as a member advocate, delivering their medications and other supplies as needed Coordinate and/or accompany members to medical, dental and social services appointments. Assist in providing coverage for other team members as needed. Engage with providers and patients to create a trusting environment. Maintain a good working relationship with all staff on the team. Attend and actively participate in all program related activities, training and meetings. Knowledge of and ability to maintain HIPAA requirements. Knowledge of and/or ability to learn basic technological skills (Word, Excel, PowerPoint, Outlook, EMR) and open to new technology Qualifications and experience: G.E.D./High School, required plus 3+ years of work experience as defined below Bachelor's degree preferred, plus 1-2 years of work experience as defined below A comparable combination of education/experience and/or training will be considered equivalent to the education listed above Experience providing direct services to unhoused individuals, preferred Experience working with community-based organizations a plus. Bilingual in Spanish/Russian a plus.

Pay:

$24.00 - $25.00 per hour
