Case Manager
Job Description
Community Housing and Shelter Services (CHSS) is seeking an empathetic, dedicated, and culturally responsive Case Manager. In this role, you will work directly with individuals and families experiencing or at imminent risk of homelessness. You will build trust with clients, assess their unique needs, help them overcome barriers to stability, and connect them with vital community housing resources.
Essential Functions Conduct Intakes:
Screen, interview, and assess participants to determine program eligibility.Develop Client Goals:
Collaborate with clients to create short- and long-term action plans for stable housing.Provide Advocacy and Referrals:
Guide clients through problem-solving and connect them to external community resources.Build Housing Networks:
Build and maintain relationships with property owners, apartment managers, and realtors.Manage Documentation:
Keep detailed, daily records, case notes, and confidential client files.Conduct Field Work As Needed:
Travel to referral agencies, program sites, and client homes for intakes and follow-ups.Qualifications Empathy and Equity:
Passion for working with diverse, low-income, or highly vulnerable populations.Autonomy:
Ability to work independently, make timely decisions, and handle sensitive data with strict confidentiality.Tech Skills:
Intermediate experience with MS Office, data entry, and web-based applications.Communication:
Clear verbal and written communication skills.Education and Experience Requirements Education:
AA degree (or working toward a BA) in Human Services, Social Work, or a related field.Experience:
1 year of experience in a public/private social services, housing, or disability agency.Note:
Equivalent, relevant work experience can completely substitute for the degree requirement on a year-for-year basis. Additional Requirements Valid California Driver's License and a clean driving record. Access to a reliable vehicle for field use (minimal). In-state auto insurance with your name listed on the policy.Pay:
$20.23 - $21.24 per hour Expected hours: 40.0 per weekBenefits:
