Full Job Description Job Summary:
Kingdom Development Inc., is seeking a dedicated and compassionate Case Manager to join our team for an opportunity in Oceanside, CA. The primary job duty is to provide case management and service coordination to formerly homeless families living in an Oceanside housing community. As a Case Manager, you will be responsible for providing support and guidance to individuals. Duties 1. Work with tenants to develop and implement an individualized service plan, goal plan or independent living plan. 2. Inform client of available community resources such as training, employment, housing, emergency food and clothing, medical, legal and recreation. 3. Organize community-building and other enrichment activities for tenants. 4. Monitor clients' progress in educational, training, and other programs. 5. Discuss clients' interpersonal relationships such as parent-child, marital, family and school. 6. Assess client needs and make referrals to other social service agencies and community service agencies. 7. Provide individualized assistance, counseling and/or advocacy to tenants, such as to assist them to access education, secure employment, secure benefits, gain skills or improve health and wellness. Included, but not limited to: Vocational/Employment Counselor, ADL or Supported Living Specialist, Substance Abuse or Mental Health Counselor, Peer Counselor, Domestic Violence Counselor. 8. Provide adult education, health and wellness, or skill building classes to include, but not limited to: Financial literacy, computer training, home-buyer education, GED classes, and resume building classes, ESL, nutrition class, exercise class, health information/awareness, art class, parenting class, on-site food cultivation and preparation classes, and smoking cessation classes. 9. Prepare and maintain accurate and complete case records, document chronological record of case contacts. 10. Prepare reports, correspondence, and memoranda. To be successful in this position one would need strong skills in A. Managing personal task lists, workspace, electronic filings, checklists, and contact lists. B. Managing time effectively, creating a rhythm, setting reminders, and focusing on the task at hand C. Communicating respectfully and professionally with co-workers, clients, and consultants in a clear and concise manner that gets things done while building relationships. D. Leading meetings with thoughtful and thorough agendas E. Typing comfortably at no less than 60 words per minute F. Using Microsoft Office programs such as Excel and Outlook G. Using reasoning skills to read, analyze and interpret governmental regulations, technical procedures, and business reports. H. Minimum Bachelor's Degree in either Social work, Psychology, Counseling or other related field. I. Experience working with mentally ill, substance abuse/addiction, and homeless population. Details a.
