- Position Available Case Manager KALOS Specialized Services is currently looking for a candidate that is outgoing, creative and has an energetic personality with a willingness to help those in need:
Job Duties Include:
EXCEPTIONAL ORGANIZATIONAL SKILLS REQUIRED!
- Setup, monitor and maintain client files
- Utilize all work designated applications, technology and equipment
- Participate in Individual Program Plan meetings
- Write thirty-day, quarterly, and annual reports on client progress
- Meet with clients to discuss progress, this includes home visits
- Communicate effectively with caseworkers, clients and families
- Participate in client activities when appropriate
- Develop and implement client objectives for ISP to be consistent with IPP
- Provide skill training in a direct care staff (Community Outreach Specialist) capacity as directed
- Provide any assistance that may pertain to the overall well being of the client
- Maintain good rapport with outside agencies and their personnel
- Comply with client Individual Program Plans
- Handle client emergencies and crisis situations
- Maintain professionalism and company dress code
- Other duties/responsibilities as directed by Operation Manager or Program Director This position may be subject to occasional on-call status.
Job Requirements:
- Bachelor's degree required in behavioral or social science or other related field and a minimum of one year experience in the field of working with the developmentally disabled. Additional experience may be substituted for a BA with the approval of the Program Director.
- Reliable vehicle to be used to transport clients
- Acceptable Department of Motor Vehicles printout. (No major violations within a 3-year period nor any prior DUI or DWI convictions)
- Personal Liability and Property Damage Insurance on the vehicle to be used
- First-aid/CPR certified or certification thereof within 30 days of hire.
