Qualifications & Requirements Bachelor's degree in social work, psychology, human services, sociology, or a related field preferred. Relevant professional and lived experience may be considered in place of a degree. At least two years of relevant experience in case management, youth services, housing navigation, homelessness services, foster-youth services, or another related field preferred. Experience working with young adults affected by foster care, housing instability, trauma, poverty, or limited family support. Experience training, coordinating, supervising, or supporting volunteers preferred. Knowledge of housing searches, rental applications, budgeting, community resources, public benefits, and common barriers to housing. Ability to build supportive relationships while maintaining clear professional boundaries. Strong organization, communication, documentation, follow-through, and problem-solving skills. Ability to manage direct participant services, volunteer coordination, and administrative responsibilities within a part-time schedule. Ability to respond calmly and appropriately to conflict, crises, and sensitive situations. Ability to work collaboratively with participants, volunteers, hosts, staff members, and community partners. Essential Duties and ResponsibilitiesHost Homes Case Management Work closely with the Host Homes Program Manager to support the effective operation of the Host Homes Program. Conduct intake interviews, assessments, and program orientations with prospective participants. Help facilitate interviews between prospective hosts and participants to support thoughtful, appropriate, and ethical matches. Develop short-term service plans that address participants' immediate needs. Work with participants to create individualized success plans addressing medium- and long-term goals. Provide ongoing case management using trauma-informed, strengths-based, motivational interviewing, and harm-reduction approaches. Help participants access housing, food, income, education, employment, healthcare, mental health services, benefits, and other community resources. Coordinate services with outside agencies and providers to support participants' needs and reduce duplication of services. Maintain regular contact with participants and monitor progress toward identified goals. Support the resolution of concerns or conflicts involving participants and hosts. Provide crisis support as appropriate and follow organizational safety, reporting, and escalation procedures. Keep the Host Homes Program Manager informed of significant participant, host, or program developments, including safety concerns, conflicts, grievances, or accountability issues. Support participants as they prepare to transition from Host Homes into stable housing. Housing Navigation Coach Program Leadership Lead the day-to-day implementation of the Housing Navigation Coach Program. Prepare approved participants for the program by assessing housing needs and barriers, reviewing program expectations, completing required worksheets, and confirming readiness to work with a coach. Train, match, and support volunteer Housing Navigation Coaches in coordination with the Volunteer Coordinator. Provide coaches with clear assignments, resources, and guidance while reinforcing confidentiality, professional boundaries, documentation, and safety requirements. Monitor participant and coach communication, assigned activities, housing-search progress, and program participation. Provide professional case-management support when participant needs or concerns exceed the volunteer coach's role. Coordinate approved housing-related assistance, referrals, and limited follow-up after placement. Maintain accurate program records and track participant services, volunteer activity, housing applications, placements, and outcomes. Gather feedback, address program concerns, and recommend improvements to strengthen service delivery. Volunteer and Program Coordination Coordinate with the Volunteer Coordinator on the recruitment, screening, onboarding, and retention of Housing Navigation Coaches. Participate in prospective coach interviews and help assess volunteers' readiness for the role. Maintain appropriate boundaries between volunteer coaching and professional case management. Monitor coach performance and address incomplete work, communication concerns, or failure to follow program expectations. Assist with coach meetings, continuing education, and recognition activities. Help maintain current program handbooks, worksheets, training materials, procedures, and resource lists. Documentation and Organizational Responsibilities Maintain timely, accurate, and complete participant files, assessments, service plans, case notes, referrals, and program records. Track participant enrollment, services, volunteer activity, housing applications, placements, and other required outcomes. Complete documentation needed for organizational, grant, and contract reporting. Protect participant confidentiality and follow all organizational policies, procedures, professional boundaries, and data-security requirements. Participate in staff meetings, case consultations, planning meetings, training, and other meetings as required. Assist with the development or facilitation of workshops and support groups for young adults. Represent Finally Family Homes professionally in collaborative networks, community meetings, and related events. Perform other related duties consistent with the responsibilities of the position.
Pay:
$27.00 per hour Expected hours: No more than 20.0 per week
