Job Description

We are seeking a compassionate, empathetic, organized, focused and quality driven Medical Case Worker who will work directly with patients, families, providers, and community agencies to coordinate care plans and connect patients with resources in achieving positive health outcomes.
Duties will include:
Conducting intake assessments and evaluation of patient's needs Coordinate medical, behavioral and social support services Assisting with insurance, referrals and healthcare resources Advocating for patients and families during treatment planning Collaborating with physicians, nurses and external agencies Supporting discharge planning and follow up care
Minimum Qualifications:
Bachelor's degree Experience working in outreach, housing navigation & Intensive Case Management Services Experience in healthcare, social work, case management, or patient advocacy professional casework experience interviewing, counseling, and assisting patients/clients with social problems A valid California Class C Driver's License or reliable transportation to carry out job-related duties Able to manage multiple cases with empathy Able to work in a Team environment Strong communication skills
Desirable Qualifications:
Experience working with individuals experiencing homelessness. Experience providing clinical care services within ARFs and RCFEs. Knowledge of complex care management services for high-need, high-cost medical beneficiaries. Familiarity with DMH, DPH, and community mental health and substance use programs and resources. Experience in assessing and managing clients with substance use disorders (SUD), including familiarity with MAT (Medication-Assisted Treatment) and Addiction Psychiatry.
Pay:
From $26.00 per hour Expected hours: 40.0 per week
