The Medical Case Worker will coordinate individualized care plans, connect patients with appropriate resources, and support patients in achieving positive health outcomes. Key Responsibilities Conduct intake assessments and evaluate patients' medical, behavioral, and social needs. Coordinate medical, behavioral health, and social support services. Assist patients with insurance navigation, referrals, and access to healthcare and community resources. Advocate for patients and families throughout treatment planning and care coordination. Collaborate with physicians, nurses, behavioral health professionals, and external community agencies. Support discharge planning, care transitions, and follow-up services. Maintain accurate and timely case documentation and communicate effectively with members of the care team. Manage multiple cases while maintaining a patient-centered, trauma-informed, and compassionate approach. Minimum Qualifications Bachelor's degree from an accredited college or university. Professional experience in outreach, housing navigation, Intensive Case Management Services, healthcare, social work, case management, patient advocacy, or a related field. Experience conducting interviews, assessments, counseling, and assisting patients or clients with medical, behavioral, or social needs. Knowledge of healthcare and community-based resources and the ability to coordinate services across multiple systems. Valid California Class C Driver's License or reliable transportation to perform job-related duties. Ability to manage multiple cases, prioritize competing responsibilities, and meet deadlines. Ability to work effectively both independently and as part of a multidisciplinary team. Strong written, verbal, interpersonal, and organizational skills. Demonstrated ability to interact with patients and families with professionalism, empathy, and respect. Desirable Qualifications Experience working with individuals experiencing homelessness or housing instability. Experience providing clinical or supportive care services within Adult Residential Facilities (ARFs) and Residential Care Facilities for the Elderly (RCFEs). Knowledge of complex care management services for high-need and high-cost healthcare beneficiaries. Familiarity with Department of Mental Health (DMH), Department of Public Health (DPH), community mental health, substance use disorder, and other community-based programs and resources. Experience assessing and supporting individuals with substance use disorders (SUD). Familiarity with Medication-Assisted Treatment (MAT) and addiction psychiatry. Experience coordinating services for individuals with complex medical, behavioral health, and social needs.
