$24.00/Hr. Catholic Charities is seeking a Case Manager in the Supportive Services for Veterans and Families (SSVF). This is a full-time position based out of our Stockton office. The Case Manager will assist low-income homeless veterans and veteran families at-risk of becoming homeless by providing case management and housing stabilization services. A candidate must have a commitment to and compassion for the needs of low-income populations. The responsibilities include Conducts in-depth assessments/reassessments covering veteran's needs and rehousing stability. Housing advocacy & intervention to assist veteran family maintain their current housing or locate affordable housing. Refers clients to other agencies for services not available within Catholic Charities.
Desired qualifications:
Preferred qualifications include knowledge of Homeless Management Information System (HMIS) data entry, Continuum of Care (CoC), Coordinated Entry System (CES). Strong verbal and written communication skills, ability to do community outreach and to community organizations and case management. Must have a clean DMV record with the ability to travel throughout San Joaquin, Stanislaus, Tuolumne, Mariposa and Calaveras counties, our service areas.
Pay:
$24.00 per hour Expected hours: 40.0 per week
