Overview Join our dedicated team as a Maternal Health Group Facilitator and play a vital role in supporting expectant and new mothers through comprehensive, culturally sensitive group programs. In this dynamic position, you will lead educational sessions, facilitate peer support groups, and coordinate community resources to empower women during pregnancy, postpartum, and early motherhood. Responsibilities Management, facilitation and organization of the Prenatal & Postpartum Group Intervention Sessions with co-facilitator. Enters data related to Group Sessions, Birth Preference, Assessments, Safety Check List, etc. in a timely and accurate manner. Coordinates and consults with Family Health Advocate (FHA) to ensure that group session goals are linked to case management goals. Maintains awareness and familiarity with local community and social services for participant referrals Maintains confidentiality and adheres to HIPAA regulations. Attend and participate in case conferencing with local BIH staff and any other members of a multidisciplinary team. Provide basic case management services to a small caseload of 15-20 case managed-only low-risk participants to include participant Assessments, Individualized Client Plan (ICP) and enhanced case management that is on-going throughout the intervention; connection of local resources, assist with basic needs, promote medical care follow-up for the participant and her child, referrals to local services as needed. follow-up and care coordination, Link Medi-Cal and Medi-Cal eligible participants to Medi-Cal programs and services, assuring access to and maintenance of medical/health and social services. Updates and maintains accurate confidential records and files; prepares monthly reports and other reports as needed. Provides feedback to

on the effectiveness of services and data forms. Attend monthly BIH and MCAH staff meetings to assist in identifying appropriate and needed services. Learns procedures for determining community health education needs; learns and assists in the planning, developing and implementing of public health educational and informational programs and services. Learns to organize and implement meetings, workshops, seminars and conferences and participates on a limited basis; learns to prepare a variety of public informational display material such as posters, exhibits and graphics. Assists in researching data and preparing written educational and informational media. Learns the structure and functioning of community agencies and organizations and their public health educational needs; assists public agencies in developing public health related educational programs and projects. Assists in providing in-service training to agency staff and community organizations; prepares and gives talks to groups. Assists in the preparation and maintenance of technical records and reports. Attends orientation sessions and reads assigned materials to become familiar with departmental policies, procedures and methods; learns organization and functions of the various divisions of the department; becomes familiar with work routing, procedural questions and related matters. Interviews clients in their homes or in the office to identify and assess their needs and problems; researches, evaluates, and collaborates with clients to develop a realistic service plan. Coordinates service plans with supervisors and other staff and agencies with which the client is involved. Interprets rules, regulations, and policy for communication to clients and the general public within the scope of responsibility; confers with supervisor regarding caseload problems.
