Entry-Level CCA Case Manager position - BSW Required-New Graduates Encouraged to

Orange County, CA Are you a recent college graduate looking to start your career in social services or healthcare? We are seeking a CCA Case Manager to support our Assisted Living Waiver (ALW) Program. This is an excellent opportunity for individuals who are passionate about helping seniors and adults with disabilities remain safely in their communities. Recent graduates are strongly encouraged to apply. What You'll Do Assist with intake and screening for the Assisted Living Waiver Program Collect client information, medical history, and insurance details Prepare and send consent forms and application documents Coordinate assessments with nurses and community partners Maintain organized and accurate client records Answer questions about program services and eligibility Qualifications Bachelor's degree required (Human Services, Social Work, Sociology, Psychology, or related field preferred) No Master's degree required Internship or volunteer experience in healthcare or social services is a plus Strong communication and organizational skills Bilingual candidates are encouraged to apply Perks Opportunity to grow into a full-time role Paid sick time 401(k) eligibility after 90 days Employee Assistance Program (EAP) Join a mission-driven team dedicated to improving the lives of vulnerable individuals in our community.
