We provide Occupational, Physical and Speech Therapy to children with disabilities ages 0 - 21.Job Description:
The Case Manager (CM) is a liaison between the parents, therapist and funding source. The CM coordinates therapy schedules for therapist within a specific territory and/or clinic along with sending and tracking patient related correspondence, client file maintenance and data entry in our EMR system. The CM is responsible for the optimal management of their assigned funding source. They communicate with the funding source, provides excellent customer service, and are in constant communication with therapists and parents for any questions, changes and concerns. The CM is always recruiting clients from the assigned funding source to achieve productivity and expansion of the caseload. The CM maintains personal aptitude in solving problems, orderliness, responsibility, calmness under deadlines, etc. The CM also completes monthly e-billing and performs Quality Assurance call to ensure our clients are receiving optimal services. Other duties as assigned.Qualifications:
- Ability to initiate and follow-up on all tasks.
- Strong verbal and written communications skills; Bilingual - Spanish is required
- Excellent customer services skills
- High Volume Scheduling experience
- Must possess personal aptitude in solving problems, orderliness, responsibility, calmness under tight deadlines
- Must be detail-orientated and thorough follow-up skills
- AA or Bachelor's degree
- 1 - 2 years of administrative experience (health care setting preferred)
- Knowledgeable with all Microsoft Word, Excel, Outlook, etc.
